Malaria is still killing people, and hospitals in third world countries don't work the way they do here. There's no EMTALA law or "care for the patient regardless of ability to pay."
He needs to get IV fluids and be observed. I can't allow my pride to get in the way of other people helping me. I don't want to have to ask, but I am tapped. So, I'm asking for help. I work 60 hour weeks just to provide. It's the way many people are having to live I'm sure. So, I'm not special, but he's so special to me.
